TATTOO CONSENT FORM

*AGE & CAPACITY

I confirm that i am aged 18 or over, have provided valid proof of age if requested, and am not under the influence of alcohol or recreational drugs. I am able to give informed consent to this procedure.

*HEALTH INFORMATION

I confirm that the health, medication, allergy and other information I have provided on this form is accurate and complete to the best of my knowledge. I have disclosed anything that may reasonably affect the tattoo procedure or healing.

*PROCEDURE & RISK

I understand that tattooing permanently alters the skin and carries risks including infection, allergic reaction, scarring, swelling, ink migration and variations in healing. I have had the opportunity to ask questions and freely consent to being tattooed.

*DESIGN & PLACEMENT

I have checked and approved the design, spelling, meaning and placement of my tattoo before the procedure begins. I understand that the finished tattoo may vary slightly from the artwork and that tattoos naturally change and fade over time.

*AFTERCARE

I understand that correct aftercare is important and agree to follow the aftercare instructions provided by 14 Arrows. I understand that once I leave the studio, following those instructions is my responsibility.

*STUDIO CONDUCT

I understand that 14 Arrows is a safe and respectful space. Harassment, threatening or inappropriate behaviour towards staff or customers may result in the session being stopped and future appointments being cancelled.
